Making your wishes known

Interesting article in the Wall Street Journal about the use of the POLST form (Physician Order for Life-Sustaining Treatment). It is intended for use by those who have a life-threatening or terminal condition and focuses on specifying the patient's wishes for use of life-prolonging measures.
In my opinion, the real take-away is that discussing your wishes for end-of-life care increases the chances that your wishes will be honored. That may seem like a given, but it's not. Less than one-third of American adults have adequately addressed the issue of advance directives.
